medication Under reviewBisphosphonates are a class of antiresorptive medications used to slow bone breakdown by inhibiting osteoclast-mediated bone resorption. They bind strongly to hydroxyapatite in bone, which helps reduce bone turnover and preserve bone mineral density over time. Common agents include alendronate, risedronate, ibandronate, and zoledronic acid. In healthy adults, bisphosphonates are primarily used to prevent and treat osteoporosis-related bone loss, especially after menopause or with aging, and to reduce fracture risk in people at high risk of fragility fractures. They are not nutritional supplements in the usual sense; they are prescription drugs with well-established bone-specific effects and a comparatively narrow clinical use profile.
Research summary
Human clinical evidence is strong that bisphosphonates increase bone mineral density and reduce vertebral and nonvertebral fracture risk in patients with osteoporosis, with effects seen across multiple randomized trials and meta-analyses. The best-supported benefits are bone health and fracture prevention, with some evidence suggesting a time-to-benefit of roughly a year for preventing nonvertebral fractures in postmenopausal women with osteoporosis. Safety is generally good in appropriate patients, but important adverse effects are documented, especially gastrointestinal irritation with oral agents, transient flu-like acute-phase reactions with some intravenous agents, and rare serious events such as atypical femur fractures and osteonecrosis of the jaw with longer-term use. Some analyses have raised concern about atrial fibrillation with intravenous zoledronic acid, but this has not been consistently confirmed across trials.
