Human research on spironolactone is extensive for approved cardiovascular indications, but in healthy or relatively healthy populations the most relevant evidence concerns off‑label dermatologic use and emerging neuropsychiatric applications. Randomized, placebo‑controlled trials in women with acne vulgaris show that oral spironolactone at doses of 50–100 mg daily improves inflammatory acne lesions and global acne severity with acceptable tolerability, supporting its role as an effective antiandrogenic therapy for female acne. Large observational analyses have also examined spironolactone use in otherwise healthy young women and girls, documenting substantial growth in prescribing over the past two decades, mainly driven by dermatologic indications.
